This application is for an individual who is not currently licensed with the Board. The Board may issue a 2-year conditional license to an individual that graduated from a Sonography, CIS/CES program and is actively working to fulfill requirements for registry with ARRT, ARDMS, CCI, or taking the ND state administered exam. Sonography Conditional Licensure Application

This application is for an individual that currently has an active primary modality with the state of ND and wishing to work toward their Sonographer Registry. Please complete the Sonography Conditional Licensure Application. There is no additional fees as this is only a tracking mechanism.

MRI Conditional Licensure Application

This application is for an individual that currently has an active primary modality with the state of ND and wishing to work toward their MRI Registry. Please complete the MRI Conditional Licensure Application. There is no additional fees as this is only a tracking mechanism.

Conditional Licensure Application Process Overview

  1. Complete the Application for Conditional Licensure and submit the necessary documents.
  2. The Application Fee is $25 and the License Fee is $150.
  3. Some of the Criminal History background check forms are NOW available online, please refer to the Crimial History Record Check section of the website. The forms MUST be returned to the office. (Incomplete forms or delays in returning this form will result in delays in becoming licensed.)

    Once the completed Criminal History Background check forms are received, they are mailed to the Bureau of Criminal Investigation (BCI) for processing. It generally takes about 2-4 weeks before those results are returned to the Board office. This CANNOT be rushed or expedited!

Note: If the background check results are clear and all documents have been received, the license may be issued.

Prior to beginning the application please ensure you have all the correct documents. Contact the Board office if you have any questions. 

  • A copy of your citizenship documentation (Social Security Card, US Passport, US birth certificate, foreign VISA or permission to work in the US). A driver's license is NOT acceptable documentation.
  • If you answered "Yes" to any of the questions in the Personal Background History, an explanation and any relevant documentation as requested at the end of that section must be submitted with the application. If you cannot send a copy of the criminal judgment, please include that in your explanation.
  • A copy of an official transcript of completing a program.
  • Proof of being registry eligible. Send a copy of passing one of the exams or copy of printout from registry's online page showing you have applied for the exam. 

To transition your Conditional to a full license:

  1. Successfully pass the registry exam.
  2. Submit a copy of your registry card (Each time you renew your registry (ARRT, ARDMS, CCI, NMTCB, etc.), you must send a copy of your new registry card to the NDMIRTB office). Conditional and Temporary License Holders will not need to complete another Criminal History background check when they transition to full licensure.

Application for Initial Licensure

If you have already graduated and received your registry exam results or registry card, or if you are already licensed or practicing in another state and wish to practice in ND permanently or as a traveler, you will complete the Application for Initial Licensure. If you are a Traveler (locums tenens) please use the Application for Initial Licensure to apply for licensure in ND.

Application for Temporary Licensure

If you are a NEW student graduating from a training program you will complete the application for a Application for Temporary Licensure. A Temporary license may be issued to new graduates in Radiography or other Board approved modality. Temporary licenses (only valid for 180 days) are issued for qualified individuals who need to take the registry exam or awaiting the exam results.
