Licensee Request for a Written Verification

This form is to request a written verification to be sent to another state board or employer. Licensees, employers, and the public may verify a license anytime via the online verification page. The online verification is a primary source verification and comes directly from the Board’s database.

Current or Expired Licensees - If you are applying for a license in another state, you may request an official verification of a license containing the Board's official seal to be sent to another state. Login below and complete the form to confirm where to send the verification. If you are an Employer/Travel Companies the board requests that you obtain this verification thru the licensee. There is no fee for this service.
