Applications

Please choose the appropriate Application to apply for ND Licensure. North Dakota State law requires the appropriate current registry(s) if actively practicing as a:

Nuclear Medicine Technologist, Radiation Therapist, Radiographer, Radiologist Assistant, Sonographer, Magnetic Resonance Imaging (MRI) Technologist, Registered Cardiovascular Invasive Specialist (RCIS), Registered Cardiac Electrophysiology Specialist (RCES) if actively practicing more than one must have appropriate current registry in each.

Continuing Education (CE) compliance begins in the 2020-2021 (January 1, 2020 - December 31, 2021) cycle. For more information available, please refer to Continuing Education section.  

 

Application Status: Once you send your application or apply online, you will be able to check your Application Status on the Board's website. The status will show you what has been received and what is still missing. The Board office does update the status a few times a week and does their best to keep it accurate. Please give the Board 2-3 days to enter your information into the database or accept your online application before you check the status. To login to the application status, you do not need a password or account. You will login with your last name, last four digits of your Social Security Number, your date of birth, and what license type you are applying for. Make sure you use the forward slashes (/) in the date of birth. If you are not able to login, please contact the Board office. Once your license has been issued, you will no longer be able to login in here and will need to verify your license on the Verification page.  You do not need a license number to verify your license.               

 
Initial / Travelers / Endorsement License Application
The Board may issue an initial license to an applicant who is currently registered with a national certifying body (ARRT, ARDMS, NMTCB, CCI, etc.).
Please check all the discipline(s) in which you are REGISTERED and currently wish to practice and apply for licensure.
Students (Temporary License) Application
Temporary licenses (only valid for 180 days) are issued for qualified individuals who need to take the registry exam or awaiting the exam results.
Conditional License Application
A ND Conditional license is issued to qualified individuals who need to take the registry exam. This will allow individuals to practice while working to fulfill requirement hours to become registry eligible.
MRI Conditional Form
(This is only a tracking mechanism with NO FEES.)
The MRI Conditional License allows you to perform MRI imaging while you work toward completing your MRI registry.
If performing MRI in ND you MUST have your MRI registry OR have a Conditional MRI license, ND state law.
Reinstatement
If previously licensed by NDMIRTB and wishing to become licensed again, please use this form and follow the instructions.
Relicensure requires completion of a Criminal History Record Check. The background check is NOT waived even if previously licensed. NDAC 114-02-02-02
LXMO
(Limited X-Ray Machine Operator)
Due to 2017 legislation, Senate Bill 2327, allows for the transition of LXMOs from regulation by the ND DoH to licensure with the NDMIRT Board. This transition is dependent on the effective date of the newly created ND Department of Environmental Quality (DEQ); at this time it is scheduled for December 31, 2018.
BD (Bone Densitometry)
Non-Imaging Professionals
Due to 2017 legislation, Senate Bill 2327, allows for the transition of Bone Densitometry Technicians from regulation by the ND DoH to licensure with the NDMIRT Board. This transition is dependent on the effective date of the newly created ND Department of Environmental Quality (DEQ); at this time it is scheduled for December 31, 2018.
